To register your child(ren) please print out the Registration Packet found to the right and complete it in full.

In order to complete the registration you will need to bring the following required documents:

• Birth Certificate
• Up-to-Date Immunizations
• Health Records
• Proof of Residency (electric, fuel, mortgage, or telephone bill dated within the 30 days of commencement of registration. A current lease agreement or current Driver's License are also acceptable.
• If applicable: Custody Documents * Foster Parent Documents * Guardianship Documents
